Sadly, hospital readmissions are all too common, so common that hospitals are being penalized for readmitting one too many patients. Recent research found that Medicare is shelling out more than $26 billion on hospital readmissions.

The result? According to CMS, out of 3,400 hospitals in the US, 76 percent were penalized in 2015.
